Mối liên quan giữa bệnh uốn ván với điều kiện thời tiết trong 10 năm (2002- 2011) tại một số tỉnh Việt Nam
Association between tetanus and weather conditions in 10 years (2002 - 2011) in some ecological provinces in Vietnam

Nghiên cứu số liệu thứ cấp nhằm mô tả thực trạng mắc và tử vong do uốn ván tại Việt Nam từ 2002-2011
và mối liên quan giữa số ca mắc uốn ván với một số yếu tố khí hậu tại Việt Nam. Phân tích tương quan
Spearman được áp dụng để tìm mối liên quan giữa số ca mắc uốn ván sơ sinh và uốn ván khác với một
số yếu tố khí hậu tại Việt nam từ 2002-2011. Kết quả nghiên cứu cho thấy tỷ suất mắc uốn ván sơ sinh
cao nhất là khu vực Tây Bắc (0,22 ca/100.000 dân/năm) rồi đến khu vực Tây Nguyên với trung bình 0,19
ca/100.000 dân/năm,và thấp nhất là đồng bằng sông Hồng với tỷ suất 0,01 ca/100.000 dân/năm. Tỷ suất
mắc uốn ván khác trung bình cao nhất tại khu vực Tây Nguyên (0,67 ca/100.000 dân/năm) và Tây Nam Bộ
(0,5 ca/100.000 dân/năm), Bắc Trung Bộ có tỷ suất mắc thấp nhất với 0,15 ca/100.000 dân/năm. Tỷ suất
tử vong trung bình do uốn ván sơ sinh và uốn ván khác cao nhất ở Tây Nguyên (0,12 ca/100.000 dân/năm
và 0,06 ca/100.000 dân/ năm). Nghiên cứu chưa tìm thấy mối liên quan có ý nghĩa thống kê giữa ca bệnh
uốn ván sơ sinh và uốn ván khác với các yếu tố thời tiết: nhiệt độ trung bình, độ ẩm không khí tương đối,
lượng mưa, số giờ nắng, tốc độ gió tại các tỉnh nghiên cứu. Riêng tại Cần Thơ, nghiên cứu chỉ ra nhiệt độ
có mối liên quan thuận (r=0,21; p=0,02) và tốc độ gió trung bình tháng có mối liên quan nghịch (r=-0,25;
p=0,01) tới sự xuất hiện ca bệnh uốn ván khác. Tại Thái Nguyên, có mối tương quan thuận giữa số mắc
UV khác với lượng mưa (r=0,18; p=0,04) và độ ẩm (r=0,23; p=0,01)
Summary:
An ecological research using secondary data
to describe the status on morbidity and mortality
due to tetanus and the relationship between
the incidence of tetanus with some climatic factors
in Vietnam period 2002-2011. Spearman
correlation analyse was applied. The results
showed that the ratio of infant tetanus was the
highest in North West (0,22 cases/100.000 inhabitants/
year), following to the ratio in Central
Highland (0,19 cases/100.000 inhabitants/
year), and the lowest was in Delta River (0,01
cases/100.000 inhabitants/year). The ratio of
other tetanus was the highest in Central Highland
(0,67 cases/100.000 inhabitants/year)
and South West area (0,5 cases/100.000 inhabitants/
year), the lowest ratio was in North
Central area with 0,15 cases/100.000 inhabitants/
year. The motality rate due to both infant
and other tetanus was highest in Central
Highland (0,12 cases/100.000 inhabitants/year
and 0,06 cases/100.000 inhabitants/year). The
study found no significant association between
the statistics of neonatal tetanus cases with tetanus
and weather factors: average temperature,
relative air humidity, precipitation, sunshine
hours, windy speed in the study provinces, except
for Can Tho and Thai nguyen provinces.
In Can tho, research indicates temperature is
related positively (r = 0.21, p = 0.02) and the
average monthly wind speed has an inverse association
(r=-0.25, p = 0.01) to the appearance
of other cases of tetanus at weak correlation.
In Thai Nguyen, there was positive correlation
between other tetanus cases and rainy (r=0,18;
p=0,04) and relative air humidity (r=0,23;
p=0,01)
