Phát triển qui trình nested PCR chẩn đoán Clostridium difficile gây tiêu chảy sau dùng kháng sinh ở người

Chúng tôi đã phát triển thành công qui trình chẩn đoán vi khuẩn Clostridium difficile (C. difficile) sinh độc
tố bằng kỹ thuật nested PCR trong phân của bệnh nhân tiêu chảy sau dùng kháng sinh tại Bệnh viện Bạch
Mai. Các thông số tối ưu cho qui trình PCR bao gồm: nồng độ mồi là 0,3 μM, nồng độ MgCl2 là 2 mM, nhiệt
độ bắt cặp 550C. Giới hạn phát hiện của qui trình nested PCR là 1,2 x 101 copies/μl. Độ nhạy và độ đặc
hiệu lý thuyết trên các chủng vi khuẩn là 100%. Trên 115 mẫu bệnh phẩm phân của bệnh nhân tiêu chảy
sau dùng kháng sinh tại Bệnh viện Bạch Mai, kết quả dương tính với phương pháp nested PCR và nuôi
cấy lần lượt là 26/115 (22,6%) và 20/115 (17,4%). So sánh với phương pháp nuôi cấy, nested PCR có độ
nhạy và độ đặc hiệu lâm sàng, giá trị dự đoán dương tính, giá trị dự đoán âm tính lần lượt là 95%, 92,6%,
73,1% và 98,9%. Độ tương đồng của hai phương pháp có tỷ lệ đồng thuận là 93% và hệ số Kappa là 0,8.
Qui trình nested PCR có khả năng phát hiện nhanh chóng, đặc hiệu và chính xác vi khuẩn C. difficile sinh
độc tố, và có thể được sử dụng như một công cụ chẩn đoán nhanh và nghiên cứu dịch tễ học sự nhiễm
của vi khuẩn C. difficile trên bệnh nhân tiêu chảy sau dùng kháng sinh tại các bệnh viện.
Summary:
We successfully developed a nested PCR assay for the diagnosis of Clostridium difficile infection in patients with antibiotics associated diarrhea at National Institute of Hygiene and Epidemiology. The optimization of nested PCR assay showed optimal parameters such as primer concentrations of 0.3 μM, MgCl2 concentrations of 2 mM and annealing temperature of 550C. Detection limit of the nested PCR assay was 1.2x101 copies/reaction. Theoretical sensitivity and specificity were 100% on clinical isolates. The nested PCR assay was then applied on 115 stool samples of patients antibiotics associated diarrhea at Bach Mai Hospital. PCR results were compared with culture results. Nested PCR technique yielded 26/115 (22.6%) positive results, significantly higher than of culture 20/115 (17.4%). Clinical sensitivity, specificity, positive predictive value, and negative predictive value for nested PCR were 95 %, 92.6%, 73.1% and 98.9%, respectively. Agreement percent between 2 methods was 93% with Kappa value 0.8. Our nested PCR assay can serve as a convenient, rapid, accurate diagnostic method to detect toxigenic Clostridium difficile from clinical samples in developing countries
