Xác định tình trạng kháng kháng sinh của vi khuẩn S.pneumoniae phân lập trên bệnh phẩm lâm sàng tại bệnh viện đa khoa Khánh Hòa năm 2008-2013

Sự xuất hiện các chủng Streptococcus pneumoniae kháng thuốc (drug-resistant Streptococcus
pneumoniae - DRSP) gây ảnh hưởng nghiêm trọng tới sức khỏe con người. Trong nghiên cứu này, 154
chủng S.pneumoniae phân lập từ các bệnh phẩm lâm sàng gây nhiễm khuẩn hô hấp cấp tính, nhiễm
khuẩn huyết và viêm màng não mủ tại bệnh viện đa khoa Khánh Hòa nhằm xác định sự lưu hành của các
chủng DRSP. Nghiên cứu sử dụng kỹ thuật PCR đa mồi và kỹ thuật nồng độ ức chế tối thiểu (MIC). Kết
quả cho thấy S. pneumoniae chỉ còn nhạy cảm với kháng sinh họ beta-lactam dạng phối hợp hoặc ở nhóm
cephalosporin thế hệ III và IV. Đã có sự tiến triển sang nhóm giảm nhạy cảm đối với penicillin G (I=50%),
imipenem (I=59,7%), meropenem (I=45,5%) và gia tăng tỷ lệ kháng đối với cefaclor (R=91,6%), ampicillin
(R=68,2%) và cefuroxime (R=59,1%). Tỷ lệ kháng tương ứng của azithromycin, tetracyclin, erythromycin,
clarithromycin và chloramphenicol là 81,8%, 80,5%, 75,3%, 72,7% và 42,9%. Có 100% các chủng giảm
nhạy cảm hoặc kháng với kháng sinh penicillin G, ampicillin và cefaclor đều mang gen ít nhất một loại gen
mã hóa cho PBPs biến đổi (pbp1a, pbp2b và pbp2x). Trong đó, tỷ lệ đồng nhiễm cả ba gen chiếm tỷ lệ từ
87,5% đến 100%. Có từ 98,4% tới 100% số chủng kháng với erythromycin, chlindamycin và tetracyclin
mang gen ermb hoặc mefa. Sự gia tăng các kiểu hình kháng kháng sinh họ beta-lactam và họ macrolid,
lincosamid và tetracycline tỷ lệ thuận với sự gia tăng các kiểu gen kháng kháng sinh (các gen pbp1a,
pbp2x, pbp2b đột biến và gen ermb mã hóa ribosom 50S biến đổi hoặc gen chuyển vị mefa).
Summary:
Recently, the appearance of drug-resistant
Streptococcus pneumoniae (DRSP) is found
causing seriouseffect on patient and community
health. This study, 154 S. pneumoniae strains
causing acute respiratory infections, septicemia
and meningitis isolated in clinical specimens
at Khanh Hoa General Hospital to determine
the prevalence of drug-resistant strains by
identifying the major genotypes responsible for
antibiotic resistant by using multi-primer PCR
technique and minimal inhibitory concentration
(MIC). Results showed that S. pneumoniae
strains were susceptible only to either the betalactam
antibiotics in combination treatment or
the cephalosporins generation III and IV. The
progression to intermediate to penicillin G (I
= 50%), imipenem (I = 59.7%), meropenem (I
= 45.5%), and increased rates of resistance to
cefaclor ampicillin (R = 68.2%) and cefuroxime
(R = 59.1%) were noted. Resistant ratios to
azithromycin, tetracycline, erythromycin,
clarithromycin and chloramphenicol were 81.8%,
80.5%, 75.3%, 72.7% and 42.9%, respectively.
100% of declined susceptible or resistant strains
to penicillin G, ampicillin and cefaclor antibiotics
was found to carry at least one gene coding for
the altered PBPs (pbp1a, pbp2b and pbp2x). The
rates of strains contained all three genes were
varied from 87.5% to 100%. There were 98.4%
to 100% of strains resisted to erythromycin,
chlindamycin and tetracycline bearing ermb or
mefa genes. The increase in resistant phenotype
to beta-lactam antibiotics and their macrolide,
lincosamide and tetracycline family were found
directly proportional to the increase in antibiotic
resistant genotypes (pbp 1a, pbp 2x, pbp 2b
mutants and ermb coding for the transformed
50S ribosom or mefa transposable gene).
