Trẻ em được chủng ngừa vắc xin ho gà kháng lại vi khuẩn Bordetella pertussis đã làm giảm đáng kể tỷ lệ
mắc bệnh. Tuy nhiên theo thống kê lâm sàng, số trường hợp bệnh ngày càng gia tăng trong những năm trở
lại đây, từ 54 trường hợp trong năm 2013 lên đến 380 trường hợp trong năm 2015. Mục tiêu của nghiên
cứu này nhằm xác định sự tái xuất bệnh ho gà có thực sự do vi khuẩn B.pertussis hay do các loài khác
như B.parapertussis và B.holmesii. Tổng cộng 73 mẫu bao gồm dịch hút khí quản, dịch hút nội khí quản,
dịch phết mũi hầu và mẫu đàm trên bệnh nhân nhập viện tại phía Nam trong năm 2015 được xét nghiệm
bằng kỹ thuật multiplex realtime PCR để xác định đồng thời 3 tác nhân B.pertussis, B.parapertussis và
B.holmesii. Kết quả cho thấy có 44 (60,27%) mẫu dương tính với B.pertussis, 1 mẫu dương tính với B.
holmesii (1,37%) và 1 mẫu đồng nhiễm giữa B.pertussis và B.holmessi (1,37%). Tất cả các mẫu âm tính
B.parapertussis và B.pertussis vẫn là nguyên nhân chính gây bệnh ho gà tại phía nam hiện nay. Bên cạnh
đó, B.holmesii đã được ghi nhận lần đầu tiên về trường hợp gây nhầm lẫn với ho gà tại Việt Nam. Trẻ em
dưới 2 tháng tuổi là nhóm tuổi chưa được tiêm ngừa nên có tỉ lệ lây nhiễm cao và đây cũng là một thách
thức lớn cho y tế công cộng hiện nay
Summary:
After the introduction of vaccines against
Bordetella pertussis in the Expanded Program
of Immunization, the rate of pertussis has
dramatically decreased. However, the number
of reported cases is increasing in recent years
based on the clinical data, which slightly grew
from 54 cases in 2013 to 380 cases in 2015.
The study aimed to determine whether the
resurgence of suspected pertussis cases was due
to B. pertussis or other species including B.
parapertussis and B. holmesii. Nasopharyngeal
tracheal aspirate, endotracheal aspirate and
sputum specimens from 73 pertussis suspected
cases in patients hospitalized in Southern
Vietnam during 2015 were tested by multiplex
realtime PCR for detecting B. pertussis, B.
parapertussis and B. holmesii, respectively.
Forty four (60.27%) specimens were positive
for the B. pertussis, 1 (1.37%) for B. holmesii
and 1 (1.37%) was found as a coinfection
of both B. pertussis and B. holmesii. No B.
parapertussis was detected and based on the
results, B. pertussis remains the main cause
for pertussis in southern Vietnam. B. holmesii
is recorded in the first time caused pertussis
like disease in Vietnam especially with a
coinfection. Children younger than 2-month
old are a high risk unvaccinated group that
posing a real challenge for public health.
