Dịch tễ học và lâm sàng bệnh ho gà trong hệ thống giám sát tiêm chủng mở rộng tại khu vực phía Nam, 2015-2017
Epidemiology and clinical of children with Pertussis in surveillance system of Expanded program on immunization in Southern Vietnam, 2015 - 2017

Nhằm đánh giá đặc điểm dịch tễ học và lâm sàng các trường hợp ho gà, chúng tôi tiến hành phân tích
mô tả loạt ca bệnh ho gà trong khu vực phía Nam (KVPN) dựa vào hệ thống giám sát và số liệu thống kê
bệnh truyền nhiễm. Từ 1/1/2015 đến 30/9/2017, KVPN ghi nhận 138 trường hợp ho gà, lưu hành chủ yếu
tại các tỉnh TP.HCM với 33 ca (0,20 trên 100.000 dân), Cần Thơ với 17 ca (0,10 trên 100.000 dân) và Bình
Phước với 14 ca (0,08 trên 100.000 dân). 100% các trường hợp mắc bệnh ho gà chưa được chủng ngừa
đầy đủ (chưa đến tuổi chủng ngừa ho gà (DTwP-HepB-Hib) hoặc chủng ngừa chưa đủ liều miễn dịch cơ
bản). Nhóm tuổi mắc chủ yếu ≤ 6 tháng (84,8%), trong đó, nhóm 2 đến 3 tháng chiếm tỷ lệ cao (41,3%),
nhóm <2 tháng chiếm 31,9%. Các triệu chứng thường gặp bao gồm ho (100%), cơn ho kịch phát (68,1%),
viêm phổi (55,8%), nôn sau ho (45,7%) và suy hô hấp (25,4%), ít gặp hơn là thở rít (13,8%), co giật (1,4%).
Việc chủng ngừa ho gà đủ mũi, đúng lịch cho trẻ và duy trì tỷ lệ chủng ngừa cao tại cồng đồng sẽ góp phần
giảm thiểu nguy cơ mắc bệnh và các biến chứng, triệu chứng nặng của bệnh.
Summary:
To evaluate the epidemiological and
clinical characteristics of pertussis cases, we
conducted a descriptive analysis of pertussis
cases series in the South of Viet Nam based on
the surveillance system of infectious diseases
and infectious disease statistics data. From
1/1/2015 to 30/9/2017, 138 pertussis cases
were reported in southern Vietnam, circulating
mainly in HCM City with 33 cases (0,20 per
100.000 population), Can Tho province with 17
cases (0,10 per 100.000 population) and Binh
Phuoc province with 14 case (0,08 per 100.000
population). 100% pertussis cases have not been
fully vaccinated (not enough age to be vaccinated
(DTwP-HepB-Hib) or partially vaccinated with
insufficient doses against pertussis). Children
aged ≤6 months were accounted for 84.4% of
the cases, of which the 2 to 3 month-old group
accounting for 41.3% and the less-than 2 monthold
group accounting for 31.9%. The common
symptoms were cough (100%), paroxysmal
cough (68.1 %), pneumonia (55.8%), vomiting
after coughing (45.7%) and respiratory failure
(25.4%), whereas less commonly symptoms
were wheezing (13.0%) and seizures (1.4%).
The timely vaccination against pertussis with
sufficient doses and the maintaining rates of
immunization rates in the community would
minimize the risk of contracting pertussis,
its complications and severe symptoms.
Concurrently, acellular pertussis vaccines (Tdap)
for pregnant women and for adults who often
contact and care infant in the first months of life
(mothers, fathers, brothers, sisters, grandparents,
caregivers) should be taken into consideration
to help newborns get pertussis antibodies from
mothers during the first months and reduce the
risk of infection from asymptomatic infection
persons.
