Nghiên cứu mô tả cắt ngang trên một loạt ca bệnh được thực hiện nhằm mô tả đặc điểm lâm sàng và thực
trạng thiếu hụt một số chất dinh dưỡng của 57 bệnh nhi dưới 5 tuổi có hội chứng ruột ngắn được điều trị tại
bệnh viện nhi Trung ương từ tháng 8/2015 đến tháng 8/2018. Kết quả cho thấy tuổi trung bình của trẻ mắc
hội chứng ruột ngắn là 5,1 ± 7,2 tháng tuổi và bệnh gặp ở trẻ trai nhiều hơn trẻ gái (68,4% so với 31,6%).
Tuổi trung bình khi phẫu thuật 23,5 ngày tuổi. Tỷ lệ trẻ có hậu môn nhân tạo (71,9%). Tỷ lệ suy dinh dưỡng
ở bệnh nhi có hội chứng ruột ngắn cao như thể nhẹ cân (96,5%), thấp còi (64,9%). Thiếu hụt vi chất dinh
dưỡng cũng cao ở bệnh nhi hội chứng ruột ngắn gồm thiếu kẽm (29,8%), sắt (19,2%), vitamin D (81%),
canxi máu (40,3%). Nghiên cứu cho thấy bệnh xuất hiện sớm trong những tháng đầu đời, với đặc trưng
kém hấp thu các chất dinh dưỡng nên tỷ lệ suy dinh dưỡng và thiếu hụt dinh dưỡng cao. Để nâng cao hiệu
quả điều trị, giảm bớt biến chứng cho bệnh nhân thì kiểm soát tốt thiếu hụt dinh dưỡng có ý nghĩa rất lớn
trong cải thiện kết quả đầu ra.
Summary:
A cross-sectional descriptive study in a series
of cases was conducted to characterize the clinical
and some nutritional deficiencies in 57 children
under age five with short bowel syndrome treated
at Hanoi National Hospital of Pediatricsfrom
August 2015 to August 2018. The result showed
that mean age of the childrenwas 5.1 ± 7.2 months
old and disease was more common in boys
than in girls (68.4% vs. 31.6%). Average age
at surgery was 23.5 days old. The rate children
had been having colostomy (71.9%). The rate
of malnutrition was high in children with short
bowel syndrome such as underweight (96.5%),
stunted (64.9%). Micronutrient deficiency also
were high included zinc deficiency (29.8%),
iron (19.2%), vitamin D (81%), hypocalcemia
(40.3%). The study found out the disease occurs
early in the first months of life; characteristics
of the disease is malabsorption of nutrients,
therefor, malnutrition and nutritional deficiencies
are high. In order to improve the effectiveness of
treatment, to reduce the complications of patients,
good control of nutritional deficiencies is very
significant in improving the outcome.
