Đánh giá phương pháp PCR phát hiện Human Papillomavirus trên mẫu bệnh phẩm sử dụng 2 bộ mồi khác nhau
Evaluation of methods using two different primer sets for detection of Human Papillomavirus in clinical samples

Human Papillomavirus (HPV) có mối liên quan đến ung thư đường sinh dục - hậu môn và các thể tiền ung thư. Có đến hơn 200 kiểu gen HPV khác nhau đã được công bố, chia thành 5 chi (genus) với sự đa dạng di truyền lớn giữa các chi, gây khó khăn cho việc chẩn đoán HPV trong bệnh phẩm. Trong nghiên cứu này, chúng tôi đánh giá phương pháp sàng lọc phát hiện HPV sử dụng bộ mồi PGMY09/11 và GP5+/6+ do Tổ chức y tế thế giới khuyến cáo trên bộ 138 mẫu chuẩn. Kết quả cho thấy phương pháp PCR với bộ 16 mồi PGMY09/11 (PGMY-PCR) và SYBR-PCR với bộ mồi GP5+/6+ (GP-PCR) có giới hạn phát hiện là 5 copies trên phản ứng đối với HPV18 (cả 2 phương pháp) và đối với HPV52 (đối với PGMY-PCR). PGMY-PCR có độ nhạy và đặc hiệu tương ứng là 95,8% và 98,5%; trong khi đó GP-PCR có độ nhạy và đặc hiệu tương ứng là 81,7% và 95,5%. PGMY-PCR không phát hiện được một số mẫu với kiểu gen HPV33,39,51,81; trong khi đó GP-PCR không phát hiện được kiểu gen HPV39,51,52, 66/68,81. Khi sử dụng đồng thời 2 phương pháp, độ nhạy được cải thiện, tuy nhiên không đáng kể so với khi sử dụng PGMY-PCR. Như vậy, PGMY-PCR có thể được sử dung để sàng lọc phát hiện các chủng HPV trong các nghiên cứu dịch tễ học trước khi tiến hành phân tích sâu hơn.
Summary:
Studies worldwide indicate that human papillomavirus (HPV) associated with ano-genital cancer and other pre-cancer stages. Up-to-date, more than 200 different HPV genotypes have been identified, classified into 5 genera with the large genetic diversity between and within genotypes, posing difficulties in HPV diagnosis in clinical samples. In this study, we evaluated 2 HPV screening methods using PGMY09/11 and GP5+6+ primer set recommended by WHO using a set of samples with known results. The results showed that the PCR method using the set of 16 PGMY09/11 primers (PGMY-PCR) and SYBR-PCR using GP5+6+ primers (GP-PCR) both have detection limit of 5 copies/reaction for HPV18 while PGMY-PCR has 5 copies/reaction detection limit for HPV52. The PGMY-PCR has sensitivity and specificity of 95.8% and 98.5%, respectively, while the GP-PCR method has sensitivity and specificity of 81.7% and 95.5%, respectively. Some HPV genotypes, which PGMY-PCR fails to detect include HPV 33,39,51 and 81 while GP-PCR fails to consistently detect HPV39, 51, 52, 66/68 and 81. Use of both methods improved the HPV detection sensitivity, but not significantly, compared to PGMY-PCR alone (specificity and sensitivity >=98.5%). Thus the PGMY-PCR method could be use as screening method for HPV in epidemiological study before further analysis.
