Một số tác nhân vi rút liên quan đến bệnh lồng ruột ở Việt Nam

Lồng ruột là trạng thái bệnh lý, khi một đoạn ruột chui vào lòng của một đoạn ruột kế cận, có thể dẫn đến
hoại tử ruột và tử vong nếu không điều trị kịp thời. Ở Việt Nam tỷ lệ mắc lồng ruột ở trẻ dưới 2 tuổi cao 4-5
lần so với các nước trên thế giới và tỷ lệ tái phát lồng ruột khá cao (10%). Nghiên cứu này tìm hiểu mối liên
quan giữa một số tác nhân đường ruột và triệu chứng lồng ruột. Với thiết kế bệnh chứng, 146 trẻ dưới 2
tuổi mắc lồng ruột (92 trường hợp lồng ruột tiên phát và 54 trường hợp lồng ruột tái phát) và trẻ nhập viện
điều trị ngoại khoa không liên quan đến vùng bụng (nhóm chứng, n=41) ở Bệnh viện trẻ em Hải Phòng và
Bệnh viện Trung ương Huế từ 2015-2016 được tuyển chọn vào nghiên cứu. Mẫu phân được thu thập và
xét nghiệm phát hiện rotavirus nhóm-A, adenovirus, enterovirus, norovirus, sapovirus và astrovirus. Kết
quả cho thấy tỷ lệ phát hiện rotavirus trong cả 2 nhóm trẻ mắc lồng ruột cao hơn tỷ lệ này ở nhóm chứng
(p<0,005). Bên cạnh đó, nhóm trẻ mắc lồng ruột tái phát có tỷ lệ phát hiện enterovirus cao hơn nhóm chứng
(p<0,05), còn tỷ lệ phát hiện adenovirus cao hơn ở nhóm trẻ mắc lồng ruột tiên phát so với nhóm chứng
(p<0,05). Nhóm trẻ mắc lồng ruột tái phát có tỷ lệ nhiễm rotavirus và enterovirus cao hơn nhóm trẻ chỉ mắc
1 lần. Như vậy một số tác nhân vi rút tại đường ruột như rotavirus, enterovirus và adenovirus đóng vai trò
quan trọng làm tăng tỷ lệ mắc lồng ruột, kể cả lồng ruột tái phát ở nước ta.
Summary:
Intussusception (IS) is the condition when
one part of the intestine inserts itself into
adjacent part, possible leading to congestion,
vascular lesions, intestinal necrosis and death if
not timely treated. In Vietnam, the prevalence of
IS among children less than 2 years of age is 4-5
times higher than those in other countries, and
the rates of recurrent episodes is also high (10%).
In this study, we investigated the correlation
of different enteric pathogens and the IS cases
(both primary and recurrent cases). In a casecontrol
design, 146 children under 2 years of
age with IS episodes (of which are 54 recurrent
cases) and the control group (children undergone
non-bowel related surgery), all from Hai Phong
children and Hue central hospitals from 2015-
2016 were enrolled in the study. Fecal samples
were collected during hospitalization and later
tested by ELISA for rotavirus group A and by
multiplex realtime PCR for adenovirus (Groups
A-F), human enterovirus, norovirus (Groups I,
II and IV), sapovirus and astrovirus. The results
showed that the detection rate for rotavirus
among IS children (one-time infection or those
with recurrent episodes) was higher compared
to the control group (p<0.005). Besides, the
children with recurrent episodes showed higher
enterovirus detection rate compared to the
control group. Infact rotavirus and enterovirus
detection rates were higher in the recurrent IS
group compared to those with single episode.
Thus enteric viruses, rotavirus, enterovirus and
adenovirus play important roles in IS cases.
