SERUM CALCIUM, PHOSPHORUS AND PARATHYROID HORMONE DISORDERS IN CONTINUOUS AMBULATORY PERITONEAL DIALYSIS PATIENT

Nghiên cứu được thực hiện với mục tiêu khảo sát nồng độ và mối liên quan giữa calci, phospho máu và PTH với các chỉ số khác ở nhóm bệnh nhân CAPD, tại khoa Nội Thận – Cơ xương khớp Bệnh viện Trung ương Huế từ 6/2013 – 8/2014. Kết quả 42 bệnh nhân suy thận mạn có 19 (45,2%) nữ và 23 (54,8%) nam, tỷ lệ nam: nữ là 1,21:1; Với thời gian lọc trung bình là 27,9 tháng. Nồng độ calci máu trung bình giảm nhẹ (2,1 ± 0,27 mmol/l), tăng nồng độ trung bình phospho và PTH máu (1,79 ± 0,53 mmol/l và 656,76 ± 389,52 pg/ml). Trung bình chỉ số Kt/Vurê tuần là 2,40 ± 0,86. Tỷ lệ bệnh nhân đạt mục tiêu theo khuyến cáo K/DOQI
2003 của calci, phospho và tích calci x phospho lần lượt là 28,6%, 35,7% và 73,8%. Tỷ lệ bệnh nhân đạt ít nhất một mục tiêu là 85,71%, đạt ít nhất 2 mục tiêu 45,24%, đạt 3 mục tiêu là 7,14% và không đạt bất kỳ mục tiêu nào là 14,29%. Khi phân loại theo nguy cơ tử vong của nghiên cứu DOPPS. Nguy cơ tử vong cao ở 35,7% bệnh nhân có phospho máu trên 6,5 mg/dl. Phospho máu tương quan thuận khá chặt với urê, creatinin máu (r = 0,652; 0,563; p < 0,001), tương quan nghịch khá chặt với Kt/V urê tuần (r = 0,526; p < 0,001).
Summary:
The study was to evaluate serum calcium, phosphorus, PTH concentrations in patients with end-stage renal disease undergoing continuous ambulatory peritoneal dialysis (CAPD) and determine the correlations of them with some factors. The cross – sectional study was used. We studied 42 patients with end-stage chronic renal failure undergoing CAPD at Nephrology Department of Hue Center Hospital from 6/2013 to 8/2014. Result of the 42 patients, 19 were females (45.2%) and 23 males (54,8%), giving a male to female ratio of 1.21:1; and average time on CAPD of 27.9 months. Average correct serum calcium level was 2.1 ± 0.27 mmol/l. Average serum phosphorus level was high (1.79 ± 0.53 mmol/l). Average serum PTH level was high (656.76 ± 389.52 pg/ml). Average weekly Kt/V urea was 2.4 ± 0.86. 28.6%, 35.7% and 73.8% cases of CKD patients met K/DOQI target value for calcium, phosphorous and calcium - phosphorous product respectively. Seven percent of the patients meet the three K/ DOQI guidelines and 14% did not meet any of the recommended guidelines by K/DOQI. Very high phosphorus levels (> 6.5 mg/dl) in 35.7% cases of patients were associated with increased all-cause mortality. Average serum phosphorus level positively correlated with serum urea, creatinine level (r = 0.652; 0.563; p < 0.001). There was a negative correlation between serum phosphorus with weekly Kt/V urea (r = - 526; p< 0.001). Disturbances in calcium and phosphorus is commonly observed in patients with CKD and associated with increased morbidity and mortality mortality in dialysis patients. Therefore, to achieve all the K/DOQI guidelines is crucial.
