INVESTIGATION OF AN OUTBREAK OF GASTROINTESTINAL INFECTION IN BEN TRE, VIETNAM, 2017

Ngày 8/11/2017, giám sát dựa vào sự kiện ghi nhận vụ dịch tiêu chảy tại một cơ sở nuôi dưỡng trẻ tại tỉnh Bến Tre, Việt Nam. Chúng tôi điều tra vụ dịch nhằm xác định tác nhân gây bệnh, nguồn bệnh và đề ra các biện pháp kiểm soát. Ca bệnh được định nghĩa là trẻ em hoặc người làm công sống tại cơ sở nuôi dưỡng trẻ có >3 lần tiêu chảy hoặc nôn trong khoảng thời gian 5/10 đến 10/11. Mẫu phân của ca bệnh và người tiếp xúc và mẫu nước được thu thập. Quan sát các hoạt động thường ngày tại cơ sở nuôi dưỡng trẻ để xác định đường lây. Trong số 94 trẻ và 7 bảo mẫu sống tại cơ sở nuôi dưỡng trẻ, 38(40%) trẻ mắc bệnh trong đó 36(95%) trẻ <5 tuổi. Không có bảo mẫu mắc bệnh. Mẫu phân của 11(52%) trong số 21 trẻ bệnh và 6(85%) trong số 7 bảo mẫu không có triệu chứng dương tính với Enteropathogenic Escherichia coli
(EPEC). Tám (100%) trong số 8 mẫu nước trước và sau hệ thống xử lý nước dương tính với EPEC. Bảo mẫu không rửa tay giữa các lần thay tã, pha sữa hay cho trẻ ăn. Nước nhiễm EPEC và thói quen vệ sinh cá nhân kém có thể là nguyên nhân dẫn đến vụ dịch. Xử lý nước và thực hành vệ sinh cá nhân tốt quan trọng để ngăn ngừa vụ dịch tiêu chảy ở trẻ em trong các cơ sở tương tự.
Summary:
On 8 November 2017, the eventbased surveillance reported an outbreak of gastroenteritis in a childcare center in Ben Tre, Vietnam. We investigated the outbreak to identify the causative agent and the source of the illness and to make control measures. Cases were defned as resident children or employees with >3 episodes of diarrhea or vomiting during 5th October to 10th November. Stool samples
were collected from cases and close contacts and water specimens were collected. Observations of routine activities in the childcare center were used to identify potential routes of transmission. Of 94 orphans and 7 nannies living in the childcare center, 38(40%) orphans met our case defnition, of which 36(95%) were <5 years old. There were no cases among nannies. Stool samples from 11(52%) of 21 ill children
and 6(85%) of 7 asymptomatic nannies tested positive for enteropathogenic Escherichia coli (EPEC). Eight (100%) of 8 water samples before and after the water flter system tested positive for EPEC. Nannies did not wash their hands
between changing diapers, mixing formula, or feeding children. EPEC-contaminated
water and poor hand hygiene practices were the likely causes of this outbreak. Adequately treated water and good hand hygiene practices are necessary to prevent outbreaks of diarrheal illness among children in institutional settings
such as this one.
