RESULTS OF THE MODEL FOR MINIMIZING THE RISK OF ERRORS IN PRESCRIPTION FOR OUTPATIENTS AT HAI PHONG INTERNATIONAL HOSPITAL IN 2019

Nghiên cứu đã tiến hành phân tích 1000 đơn thuốc ngoại trú được kê trong năm 2018 và năm 2019 nhằm
mô tả kết quả mô hình giảm thiểu nguy cơ sai sót thuốc trong kê đơn thuốc ngoại trú tại bệnh viện đa khoa
Quốc tế Hải Phòng năm 2019. Kết quả nghiên cứu cho thấy 100% số đơn thuốc được kê phù hợp với quy
chế kê đơn. Tỷ lệ đơn thuốc sử dụng thuốc hô trợ, vitamin và khoáng chất giảm gần 50%. Tỷ lệ đơn thuốc
thiếu hướng dẫn sử dụng giảm mạnh ở cách dùng, liều dùng. Tỷ lệ đơn thuốc bẻ, nghiền đối với thuốc
không được bẻ, nghiền giảm gần như tuyệt đối. Tỷ lệ tương tác thuốc gặp phải giảm từ 29,8% còn 17,8%,
trong đó tương tác mức độ chống chỉ định giảm từ 6,2% còn 0,4%. Tỷ lệ sai sót về thuốc và thời gian/
khoảng thời gian điều trị cũng giảm (sai thuốc: giảm 18,1%; sai thời gian/khoảng thời gian điều trị: giảm
25%). Nghiên cứu cho thấy các sai sót trong kê đơn ngoại trú có thể được hạn chế bởi mô hình giảm thiểu
sai sót trong kê đơn ngoại trú bao gồm các can thiệp từ việc hô trợ của các dược sĩ lâm sàng cho đến áp
dụng các cảnh báo và tiện ích khi kê đơn
Summary:
The study is conducted retrospectively on
1,000 outpatient prescriptions in 2018 and 2019
to describe results of the model for minimizing
the risk of errors in outpatient prescriptions
at Hai Phong International Hospital. Results
We found that 100% of prescriptions met
the prescription standards. The number of
prescriptions using supplement drugs, vitamins
and minerals decreased by nearly 50%. The
proportion of prescriptions lacking instructions
on drug administration and dose regimen
decreased sharply. The prescriptions calling
for breaking or crushing the pills when it is not
advisable to do so have almost disappeared.
The percentage of preseciptions where drug
interactions are found decreased from 29.8%
to 17.8%, and percentage of prescriptions
with contraindications decreased from 6.2%
to 0.4%. The rate of drug error and treatment
time/interval also decreased (the percentage
of prescriptions with wrong drug decreased by
18.1%, the percentage of prescriptions with
wrong time/duration of treatment decreased by
25%). This study showed that errors in outpatient
prescribtions could be limited by an error
mitigation model that includes interventions
ranging from the support of clinical pharmacists
to application of drug interaction warnings and
form of instructions for use when prescribing.
