Rối loạn phổ tự kỷ (RLPTK) là một rối loạn phát triển ở trẻ em, và có xu hướng gia tăng trên thế giới và
tại Việt Nam. Nghiên cứu nhằm xác định tỷ lệ hiện mắc RLPTK và mô tả một số yếu tố nguy cơ trước và
trong khi sinh ở trẻ 18 – 30 tháng tuổi tại 3 tỉnh miền Bắc Việt Nam năm 2017. Nghiên cứu cắt ngang, sử
dụng MCHAT 23 để sàng lọc nguy cơ RLPTK trên 17.277 trẻ 18 – 30 tháng tuổi tại 3 tỉnh thành miền Bắc.
Sử dụng DSM-IV để chẩn đoán RLPTK ở toàn bộ trẻ có kết quả sàng lọc dương tính và khoảng 2% trẻ có
kết quả sàng lọc âm tính với MCHAT 23. Tỷ lệ hiện mắc RLPTK ở trẻ 18 – 30 tháng tuổi tại địa bàn nghiên
cứu là 0,75%. Tỷ lệ RLPTK có sự khác biệt có ý nghĩa thống kê ở trẻ có người thân mắc rối loạn tâm thần
hoặc khuyết tật bẩm sinh; mẹ từng sảy thai, thai chết lưu hoặc nạo hút thai trước khi sinh trẻ; trẻ đẻ có can
thiệp sản khoa, sinh thiếu tháng, đẻ nhẹ cân và ngạt sau sinh. Nghiên cứu đề xuất cần theo dõi những trẻ
có yếu tố trước sinh và trong khi sinh bất thường để sàng lọc và chẩn đoán RLPTK sớm.
Summary:
Autism spectrum disorders (ASD) is a
developmental disorder among children, which
are increasing worldwide and in Vietnam.
This study aims to determine the prevalence
of autism spectrum disorders and some related
factor describe among children aged 18 and
30 months in Nothern Vietnam, 2017. This
was a cross-sectional study, using MCHAT
23 to screen children with high risk of ASD,
among 17,277 children aged 18 and 30 months
in Northern Vietnam. 100% of MCHAT 23
positive cases (255 cases) and 2% negative
cases (340 cases) were diagnosed using DSMIV criteria. The overall prevalence of ASD
among children aged 18 and 30 months in
the three study sites was 0.75%. The odds of
having ASD were signifcantly higher among
those with relatives with psychiatric disorders;
mothers with miscarriage, stillbirth, and
induced abortion history; and assisted delivery,
prematurity, low birth weight, and neonatal
hypoxia. The study has recommended that
children with abnormal prenatal and perinatal
history should be monitored and obtained early
ASD screening and diagnosis.
