ANTIMICROBIAL RESISTANCE AT HAI PHONG INTERNATIONAL HOSPITAL IN 2021

Nghiên cứu hồi cứu nhằm mô tả thực trạng kháng kháng sinh tại bệnh viện đa khoa Quốc tế Hải Phòng
năm 2021 từ 2429 mẫu bệnh phẩm có kết quả vi khuẩn dương tính. Kết quả cho thấy tác nhân gây nhiễm
khuẩn hay gặp nhất là E. coli (24,9%), tiếp theo là Strep. agalactiae (24,7%) và S. aureus (12,2%). Vi khuẩn
chủ yếu được phân lập từ mẫu bệnh phẩm nước tiểu, dịch sinh dục và mủ. Vi khuẩn E. coli đã kháng
với trimethoprim/sulfamethoxazol (67,3%), cefuroxim (55,1%), cefotaxim (51,1%), ciprofloxacin (45,2%) và
levofloxacin (43,6%). Tỉ lệ sinh ESBL của chủng E. coli lên tới 62,5%. Các vi khuẩn Gram âm gây bệnh
đường hô hấp như M. catarrhalis và Haemophilus spp. đã có tỉ lệ kháng rất cao với các kháng sinh đường
uống được sử dụng rộng rãi trong cộng đồng như amoxicillin/clavulanat (47,0 - 100%), nhóm macrolid
(79,6%). 77,3% chủng tụ cầu đã kháng methicillin và tỉ lệ tụ cầu vàng kháng clindamycin cũng lên tới
81,8%. Tỷ lệ Strep.pneumoniae kháng với các kháng sinh đường uống rất cao như levofloxacin (100%),
erythromycin (98,2%) và cefuroxim (81,8%).
Summary:
Retrospective study describing the current
situation of antibiotic resistance at Hai Phong
International Hospital in 2021 from 2429
positive bacterial specimens. The results showed
that the most common infectious agent was E.
coli (24.9%), followed by Strep. agalactiae
(24.7%) and S. aureus (12.2%). Bacteria are
mainly isolated from urine, genital fluid and puss
specimens. E. coli was resistant to trimethoprim/
sulfamethoxazol (67.3%), cefuroxime (55.1%),
cefotaxime (51.1%), ciprofloxacin (45.2%) and
levofloxacin (43.6%). The ESBL production
rate of E. coli strain was up to 62.5%. Gram
- negative bacteria causing respiratory
infection diseases such as M. catarrhalis and
Haemophilus spp. had a markable rate of
resistance to oral antibiotics widely used in the
community such as amoxicillin/clavulanate
(47.0 - 100%), macrolide group (79.6%). S.
aureus is 77.3% resistant to methicillin and the
rate of clindamycin resistance of S. aureus is
notable of 81.8%. The rate of Strep. pneumoniae
resistant to oral antibiotics is very high including
levofloxacin (100%), erythromycin (98.2%) and
cefuroxime (81.8%).
