Lưu hành của chủng Beijing hiện đại trong vi khuẩn Mycobacterium tuberculosis dòng Beijing thuộc các mức độ kháng thuốc khác nhau ở Việt Nam
Prevalence of modern Beijing genotype among clinical Mycobacterium tuberculosis isolates of Beijing lineage of different drug resistance levels in Vietnam

Tổng số 215 chủng Mycobacterium tuberculsosis dòng Beijing phát hiện được trong số 350 chủng M.
tuberculsosis có mức độ kháng thuốc khác nhau thu thập từ bệnh viện tuyến Trung ương khu vực miền
Bắc và miền Nam, 2005-2008, được phân loại bằng multiplex PCR để phát hiện chủng Beijing hiện đại,
trong đó có chủng Beijing-W, và chủng Beijing cổ điển. Kết quả cho thấy tỷ lệ chủng M. tuberculosis Beijing
hiện đại trong số chủng dòng Beijing không có mối tương quan ý nghĩa với mức độ kháng thuốc. Ở miền
Nam, chủng Beijing hiện đại lưu hành nổi trội hơn so với chủng Beijing cổ điển trong số chủng Beijing
(81,7% và 18,3%, p<0.0001), ở miền Bắc, sự khác biệt không có ý nghĩa thống kê (46,1% và 53,9%,
p>0.05). Tỷ lệ chủng Beijing hiện đại trong số chủng Beijing ở miền Nam cao hơn có ý nghĩa so với miền
Bắc (OR = 5,2 [95%CI (2,8 - 9,7)], p<0,0001). Nghiên cứu cho thấy mức độ lưu hành của chủng Beijing
hiện đại có thể là một yếu tố quan trọng quyết định tỷ lệ lưu hành bệnh lao mỗi khu vực ở Việt Nam. Chủng
Beijing-W được xác định liên quan đến nhiều vụ dịch bệnh lao kháng thuốc trên thế giới lại hiếm gặp ở Việt
Nam, và không có kiểu hình đa kháng. Điều này cho thấy chủng Beijing-W không có vai trò đáng kể trong
dịch tễ học bệnh lao ở Việt Nam.
Summary:
A total of 215 Mycobacterium tuberculosis
isolates of Beijing lineage detected among 350
clinical M. tuberculosis isolates of different
drug resistance levels, collected from reference
hospitals of North and South Vietnam, 2005-
2008, were studied. The isolates were genotyped
by multiplex PCR to classified into modern
Beijing group (MBG) and ancient Beijing group
(ABG). The W-Beijing, a specific genotype that
belongs to MBG was also identified by the same
PCR. Results showed that the proportion of
MBG isolates among Beijing isolates was not
correlated with the level of drug resistance. In
the South, MBG isolates were more prevalent
among Beijing isolates compared to ABG
isolates (81,7% vs. 18,3%, p <0.0001), In the
North, the difference was not significant (46,1%
vs. 53,9%, p>0.05). The proportion of MBG
isolates among Beijing isolates in the South was
significantly higher than that in the North (OR
= 5,2 [95%CI (2,8 - 9,7)], p<0.0001). The study
suggests that the prevalence of MBG isolates
might be an important factor that determined
the local prevalence of tuberculosis in Vietnam.
The W-Beijing strains identified responsible
for multiple disease outbreaks in the world was
rarely found in Vietnam, and didn’t have multidrug
resistance phenotype. This indicated that
the W-Beijing strains did not play a significant
role in the tuberculosis epidemiology in
Vietnam.
