Thực trạng và một số rào cản trong cung cấp dịch vụ sản khoa thiết yếu cơ bản và toàn diện tại Việt Nam
Situation and barriers to essential and comprehensive emergency obstetric care in Viet Nam

Cung cấp dịch vụ cấp cứu sản khoa thiết yếu (CCSKTY) ở tuyến chăm sóc sức khỏe ban đầu có thể làm
giảm tử vong mẹ. Tuy nhiên, việc cung cấp dịch vụ đặc biệt là CCSKTY còn có sự khác biệt giữa các vùng
miền tại Việt Nam. Bài báo này nhằm mô tả thực trạng cung cấp dịch vụ cấp cứu sản khoa thiết yếu và toàn
diện và mô tả một số rào cản trong triển khai cung cấp dịch vụ CCSKTY tại Việt Nam. Thiết kế nghiên cứu
sử dụng phương pháp định tính sử dụng tổng quan 80 tài liệu thứ cấp và phỏng vấn sâu/thảo luận nhóm 65
cuộc tại Hà Nội, Bắc Giang và Đồng Tháp từ tháng 3-8/2011. Kết quả cho thấy tỷ lệ bao phủ CCSKTY toàn
diện tăng, tỷ lệ bệnh viện huyện có mổ đẻ và truyền máu tăng lần lượt từ 63% và 47% (2007) lên 68.2%
và 59.8% (2010), tỷ lệ bao phủ CCSKTY cơ bản cũng tăng đáng kể. Một số khó khăn bao gồm một số nội
dung chính sách còn chưa được cập nhật và phù hợp với thực tế, thiếu nguồn lực và thiếu thuốc tại tuyến
xã và huyện. Do vậy để nâng cao hiệu quả trong cung cấp dịch vụ CCSKTY, cần cập nhật chính sách cho
phù hợp với tình hình thực tiễn, tăng cường thu hút nguồn lực và bổ sung thuốc thiết yếu cho tuyến huyện
khu vực vùng sâu vùng xa nhằm giảm khoảng cách tử vong mẹ giữa các vùng miền.
Summary:
Emergency obstetric care at primary
health care level can be considered
as a strategy for reducing maternal
mortarity ratio (MMR). However, there
are discrepancies in maternal mortality
and service provision among regions in
Vietnam. The aims of this article was to
describe the situation of service deliveries
related to essential and comprehensive
emergency obstetric care in Vietnam and
to analyse some factors and difficulties
related to delivery emergency obstetric
care services in Vietnam. Qualitative
research method was employed, using
literature review 80 documents and indepth
interviews/ focus group discussion
with 65 key informants were conducted at
Hanoi, Bac Giang and Dong Thap province
from March-August, 2011. Results showed
coverage of comprehensive emergency
obtestric care (EMOC) increased. District hospital can provide C-section and blood
transfusion increased from 63% and
47% (2007) to 68.2% and 59.8% (2010)
respectively. Coverage of essential
EMOC also increased significantly.
Some difficulties related implementation
including not up to date policy, lack of
human resources at district level, lack of
medicine at commune level and lack of strict
monitoring and evaluation mechanism for
the policy to be implemented from central
to grassroots level. Hence, policy makers
should update the policy (QD 385/2001/
QD-BYT) in accordance with the context
of the current health system, improve and
strengthen district and commune level
through investing on human resource and
medicine, especially for mountainous and
remote areas.
