Emergence of entorobacteriaceae carrying new delhi metallo-beta-lactamase-1 gene among inpatients admitted to Thanh Nhan hospital in Hanoi, Vietnam, 2010-2014

Vi khuẩn Gram âm mang gen New Delhi metallo-beta-lactamase 1 (NDM-1) đang là một vấn đề nghiêm
trọng tác động đến sức khoẻ trên toàn cầu. Trong nghiên cứu này, toàn bộ 98 chủng Vi khuẩn đường ruột
(VKĐR) kháng carbapenem phân lập tại bệnh viện Thanh Nhàn giai đoạn 2010-2014 được sử dụng để
phát hiện sự có mặt của gen NDM-1 và đặc tính kháng kháng sinh. Kết quả cho thấy có 23/98 (23,5%) số
chủng thuộc 4 loài VKĐR mang gen NDM-1, trong đó Klebsiella spp. chiếm tỷ lệ cao nhất 14/23 (60,86%);
Citrobacter spp. 6/23 (26,08%); E. coli 2/23 (8,69%); Trực khuẩn gram âm 1/23 (4,34%). Trong 4 khoa phát
hiện có bệnh nhân dương tính với NDM-1: khoa Hồi sức chiếm tỷ lệ cao nhất 69,6% (16/23) tiếp đến là Tiết
niệu 17,4% (4/23); Khoa Nội 8,7% (2/23) và Ngoại Thần kinh 4,3% (1/23). Tỷ lệ dương tính cao được phân
lập từ các mẫu liên quan đến đường hô hấp (dịch họng, đờm, dịch phế quản) là 13/23 (56,2%) và nước tiểu
6/23 (26,08%). Độ tuổi trung bình của các bệnh nhân dương tính là 50 tuổi (từ 19-96 tuổi), tỷ xuất chênh
về giới tính Nam/Nữ là 2,83. Tất cả các chủng mang gen NDM-1 đều kháng lại với các kháng sinh được
thử nghiệmngoại trừ một chủng Citrobacter spp. phân lập năm 2014 kháng trung gian với imipenem. Đây
là báo cáo đầu tiên về sự xuất hiện VKĐR mang gen NDM-1 tại bệnh viện Thanh Nhàn.
Summary:
The emergence and spread of Gramnegative
bacteria carrying New Delhi metallobeta-
lactamase-1 (NDM-1) gene has been
identified as a public health threat. In this study,
we investigated the presence of NDM-1 gene
andassociated antimicrobial susceptibility patterns
in98 carbapenem-resistant Enterobacteriaceae
isolates taken from patients admitted to Thanh Nhan
hospital during 2010-2014. We identified 23/98
isolates (23.5%) belonging to 4 Enterobacteriaceae
species carrying NDM-1 gene. Of which, Klebsiella
spp. was most frequently found,14/23 (60.86%),
followed by Citrobacter spp. 6/23 (26.08%); E.
coli 2/23 (8.69%) and Gram-negative bacteria,
1/23 (4.34%). NDM-1 positive isolates were
obtained from 4 departments, with highest
proportion from ICU, 69.6% (16/23) followed
by the Departments of Urology, 17.4% (4/23);
Internal Medicine, 8.7% (2/23); and Neuro
Surgery, 4.3% (1/23). Samples taken from
respiratory tract (sputum, throat and bronchial
fluids) had the highest proportion of NDM-
1 positive, 13/23 (56.2%), followed by urine
samples, 6/23 (26,08%). The median age of
patients with NDM-1 positive bacteria was
50 years old (range from 19 to 96). The maleto-
female ratio was 2.83. All NDM-1 positive
isolates were resistant to all tested antibiotics,
except one Citrobacter isolate thatwas resistant
to imipenem at intermediate level. This study
for the first time, reported the emergence of
Enterobacteriaceae carrying NDM-1 gene in
Thanh Nhan hospital.
