Một số đặc điểm lâm sàng và cận lâm sàng ở phụ nữ có thai bị viêm ruột thừa ở Việt Nam
Clinical and subclinical features of pregnant women with appendicitis at the Vietnam - German friendship hospital

Nghiên cứu tiến cứu được thực hiện nhằm mô tả đặc điểm lâm sàng và cận lâm sàng ở phụ nữ có thai bị
viêm ruột thừa. Đối tượng nghiên cứu gồm tất cả những thai phụ được mổ viêm ruột thừa tại Bệnh viện
Hữu nghị Việt Đức và được điều trị sau mổ cắt ruột thừa viêm tại Bệnh viện Phụ sản Trung ương trong thời
gian từ năm 2013-2014. Kết quả cho thấy các triệu chứng thường gặp viêm ruột thừa khi có thai là đau
bụng hố chậu phải (98,8%), nôn và buồn nôn (51,8% ), rối loạn tiêu hóa (28,2% ), đau bụng vùng thượng
vị (32,9%) trong đó liên quan giữa đau bụng vùng thượng vị gặp 3 tháng cuối (53,6%). Liên quan giữa đau
khắp bụng có biến chứng viêm phúc mạc ruột thừa (57,3%). Phản ứng thành bụng hố chậu phải (90,6%),
điểm đau Mac Burney (58,8%), cảm ứng phúc mạc (18,8%), bụng chướng (18,8%), dọa sẩy trước lúc
mổ cắt ruột thừa viêm (0%), dọa đẻ non và chuyển dạ (14,3%). Kết quả siêu âm ổ bụng: Ổ bụng có dịch
(42,4%), kích thước ruột thừa to ≥6mm (58,8%), áp xe ruột thừa (2,4%), hình ảnh thâm nhiễm mỡ xung
quanh (58,8%). Nghiên cứu này cho thấy khi nghi ngờ viêm ruột thừa ở phụ nữ có thai cần làm thêm chụp
cộng hưởng từ (MRI) ổ bụng hoặc mổ nội soi thăm dò để rút ngắn thời gian chẩn đoán nhằm làm giảm các
biến chứng do viêm ruột thừa lên mẹ và thai nhi.
Summary:
A prospective study was performed to describe
clinical and subclinical features of pregnancy
women with appendicitis at the Vietnam-German
Friendship Hospital and having the treatment for
postoperative inflammation appendectomy at the
National Obstetric Gynecology Hospital 2013-
2014 year. The results showed that some common
symptoms of appendicitis were abdominal pain
(pelvis pain) during pregnancy (98.8%), nausea
and vomiting (51.8%), dyspepsia (28.2%), and
upper abdominal pain (32.9%). The percents of
upper abdominal pain occurred in the first, second,
and third trimesters of pregnancy were 17.9%,
28.6%, and 53.6%, respectively. The percents
of stomach aches peritonitis with complicated
appendicitis and uncomplicated one were 57.3%
and 42.9%. The percents of abdomen pelvis reaction,
pain at Mac Burney’s point, induced peritonitis,
abdominal distention, risk of miscarriage prior to
surgical appendix inflammation, and risk of
premature labor were found to be 90.6%, 58.8%,
18.8%, 18.8%, 0% and 14.3%, respectively. To
abdominal ultrasound results, the percents of
abdomen having fluid, abnormal appendix with
size ≥ 6mm and normal appendix with size < 6
mm were found to be 42.4%, 58.8%, and 14.1%,
respectively. The percent of unseen appendix on
ultrasound, abscessed gut, and fatty infiltration
was determined to be 27.1%, 2.4%, and 58.8%,
respectively. The sutdy showed that when being
suspected with appendicitis, pregnant women
should have some extra MRI and/or exploratory
laparoscopy to shorten the diagnosis. This would
help to reduce later complications of appendicitis
to mother and fetus.
